Each question has five levels for you to choose from.
(1) None: No issues with this item
(2) Mild: Mild occurrence of this issue
(3) Moderate: Moderate symptoms of this item
(4) Severe: Significant symptoms of this item
(5) Extreme: Symptoms have reached a very serious level
